Final Forms Now Open for 2023-2024 School Year
Final Forms is now open for the 2023-2024 school year. All online forms can be completed and physicals can be uploaded. Please note that all forms and physicals must be completed prior to participation on June 15, 2023.
Physicals
Please note the AD Department is now using FinalForms for all athletic forms.
Physicals and online consent forms for all players (not just 9th graders) must be completed and on file with the ZCHS athletic department prior to summer training participation (June 15, 2023).
The physicals must have been on or after April 1, 2023 to be valid for the summer training sessions and the regular 2023-2024 soccer season. All physicals must be turned into the ZCHS Athletic Office (online). I can’t collect or turn physicals into the ZCHS Athletic Office. Physicals must be on IHSAA forms and ZCHS athletic consent forms must be done through the schools website.

Physicals must be signed (not stamped) by the doctor, have the doctor’s ID number, and be on the IHSAA Physical form.
