Physicals and Forms – New Website
Please note the AD Department is now using FinalForms for all athletic forms. Rank One is no longer used.
Physicals and online consent forms for all players (not just 9th graders) must be completed and on file with the ZCHS athletic department prior to summer training participation (June 1, 2022).
The physicals must have been on or after April 1, 2022 to be valid for the summer training sessions and the regular 2022-2023 soccer season. All physicals must be turned into the ZCHS Athletic Office. I can’t collect or turn physicals into the ZCHS Athletic Office. Physicals must be on IHSAA forms and ZCHS athletic consent forms must be done through the schools website.

Please note the specific instructions and requirements for the physical. Instructions for logging onto FinalForms are on this page.
Physicals must be signed (not stamped) by the doctor, have the doctor’s ID number, and be on the IHSAA Physical form.
